We have an exciting opportunity for a Community Rehabilitation Officer to join the Home First Team based at Myos House, Extra Care Housing for people living with Dementia. Our aim is to ensure that a person can remain at home and feel confident to live independently as possible. We work in partnership with other health and social care professionals to provide a person-centred service, to prevent avoidable admissions to hospital and long-term care.

Home First- Myos House ‘helps people do things for themselves’ rather than ‘doing it for them’ through support programmes that enable people to optimise their daily living skills and well-being. The role will involve working with individuals and their circle of support to maintain their confidence and continue living well with Dementia - based on their abilities and strengths and those of their family and community.
We are on site 24/7, 365 days to provide support for their assessed needs and to attend in emergency situations. As a Community Rehabilitation Officer, you will need to be solution focused, able to motivate others, have excellent decision-making and interpersonal skills, and be able to work autonomously in complex situations. With support from the Senior Officer, you will be leading the shift and be on hand to support the Community Reablement Assistants.
Training will be provided to support the successful candidate in the role, which will include medication and moving and handling.
Post holders will be expected to be flexible in undertaking the duties and responsibilities attached to their post and may be asked to perform other duties, which reasonably correspond to the general character of the post and are commensurate with its level of responsibility including direct personal care.
