We are looking for an enthusiastic and motivated individual to make a positive impact in people’s lives by working in partnership with service users, their carers, and families.
You will be a core member of a multi-disciplinary Baywide Health, Wellbeing and Specialist Services Team and contribute to the effective assessment and discharge planning with young adults, with a specialist focus on people experiencing mental health issues, in the area of Torbay and for Torbay residents in out of area hospitals in the region.
Your responsibilities will include:
1. Undertaking holistic social care assessments in partnership with service users, families, and carers.
2. Preparing care plans and commissioning services to ensure the most beneficial and cost-effective care packages are established, delivered, monitored, and reviewed.
3. Complying with all statutory requirements for Torbay and South Devon NHS Foundation Trust policies and guidelines.
4. Having clear responsibilities for the safeguarding of adults and children and operating in a manner consistent with Human Rights legislation.
5. Practicing in compliance with social care legislation including Care Act 2014 and Mental Capacity Act 2005.

You will be working within the Baywide Specialist Services Community Social Care Teams who are a supportive team that enjoys mentoring team members and students and is passionate about delivering quality care. The Baywide Specialist Services team consists of:
* The Review and Insight Team
* Transitions Team (preparing for adulthood)
* Sensory Team Services
* Disability Information and Accessible Information Services
You will also establish strong links with the specialist Adult Mental Health social care team, who will provide you with support and professional supervision and guidance to ensure high standards of practice and quality of care.
