The Care Coordinator will be responsible for care coordination, case management, communication, home care, and customer service tasks on a day-to-day basis. This job role involves visiting clients across the counties of Hampshire, Surrey, London and Sussex. Case managing clients and liaising with live in carers. Writing care plans, completing risk assessments and contributing to ensuring at all time our clients have the right match of a carer.
