JoinTeamEliot An exciting opportunity has arisen to join expanding team in our UGI CNS team for a Band 6 18-month fixed term contract. This post will provide a specialist service to UGI Cancer patients throughout their care pathway including the overarching responsibility for the support of patients living with and beyond cancer. Act as a key worker and point of contact and a professional resource for patients, their carers and the multi professional team relating to UGI cancers to ensure an expert Clinical Nurse Specialist within our Nursing Service that provides specialist nursing care for all patients, including those with complex symptom management issues. Working trust wide in a variety of settings including outpatient clinics and inpatient wards. Implemented by the post holder and delegated to other members of the team. Manages clinical caseload on an ongoing basis including making modifications within prescribed parameters reporting back to the Consultant. Autonomously performs specialist nurse procedures in line with Trust protocols and NMC guidance. Provides advice in relation to the care of individual or groups of patients. Liaises with other nursing staff caring for clients within the specialty in order to provide specialist knowledge to optimise patient care. Responsible for the effective management of allocated resources. Responsible for procurement and maintenance of physical assets and supplies for the service. Represents the Trust at external events, networking with professional colleagues to maximise the opportunities of promoting the Trust and benchmarking practice with peers. Involvement in clinical audit 24-hour GP notification audit. 25 Appropriateness & timeliness of urgent suspect cancer referrals. Assist in the gathering of information for ongoing audit projects. Co-ordination of patient’s pathway. Monitoring of MDT standards in conjunction with MDT co-ordinator. Developmental opportunities to undertake further progression and commence 2 week wait clinics. Provide advice and guidance to all levels of medical, nursing, and allied health professionals and also non-professional carers who are supporting patients with UGI cancers. Participate/ organise meetings for Upper GI groups, to enhance the development of Upper GI conditions across Primary and Secondary health care. Provide educational sessions to multi-disciplinary groups (Medical staff, Physiotherapists, Pharmacists, Occupational therapists and external interest groups). Project management – improved communication between primary and secondary care Undertake clinical trials. Develop education programmes to patients, carers and other nursing staff. Promote the development of a climate which is conducive to lifelong learning and evidence-based practice. Continually seek to improve the quality of education delivered by the team by seeking the opinions of others and formal evaluation, amending the programme as necessary. Act as a role model exploiting at every opportunity to enhance the knowledge of others in issues relating to the specialism. Provides advice which contributes to the care, well-being or education of patients/clients including health promotion. Is aware and adheres to Trust policies and procedures, the Health and Safety Act Maintains confidentiality at all times in line with the Data protection Act (2000) Opportunity to complete health assessment. Function as a key member of the Multidisciplinary Team, working closely with the Medical/Oncology Consultants, community teams including district nurses and community palliative care teams. Liaise with lead cancer nurse on matters relevant to the role, practice delivery, up to date guidelines and professional issues, incorporating clinical supervision. Provide services to facilitate and maintain the optimum quality of life for patients and relatives during investigations, throughout the course of the illness including palliative care.
