1. Providing triage, comprehensive specialist assessment, review and evaluation of the needs of patients and carers
2. Being responsible for planning programmes of care to promote health gain and maximise independence, actively case managing patients through their journeys
3. Allocating appropriate workloads to members of the Rapid Response & Intermediate Care (RRIC) team
4. Working with a range of professionals in order to ensure care is appropriate and delivered in a timely manner.
5. Demonstrating professional skills and competence in leadership, care planning / co-ordination, and care and treatment delivery. Support the development of clinical pathways

Job responsibilities
To provide active case management programmes for patients with complex needs within the locality and to work with the Advanced Community Practitioners to allocate appropriate workloads to members of the Rapid Response & Intermediate Care (RRIC) Team.The post holder will participate in rotation to the North or South Squad rota as part of their role. The Community Practitioner will work with a range of professionals in order to ensure care is appropriate and delivered in a timely manner. The post holder will be able to demonstrate their professional skills and competence in leadership, care planning / coordination, medicines management, and care and treatment delivery.

SKILLS, ABILITIES & KNOWLEDGE
Essential
6. Valid and current driving licence and car to be able to travel to locations throughout Buckinghamshire.
7. Demonstrate profession specific clinical assessment planning delivery and evaluation skills.
8. Ability to motivate staff.
9. Excellent written and verbal communication skills.
10. Self-Awareness
11. Presentation skills

12. Ability to manage complex cases.
13. Ability to develop and maintain partnership working.
14. Knowledge of legal requirement for working with vulnerable adults and those who lack capacity.

Experience
Essential
19. Demonstrate experience post or pre-registration placements in relevant clinical areas for therapists:- Care of Elderly Orthopaedics Neurology Respiratory Palliative Care Long term conditions
20. In depth experience of working within multi-agency/multi-disciplinary team
Desirable
21. Experience participating in research or audit project.
22. Experience of day-to-day staff management case allocation to Health Care Assistants
23. Experience of electronic clinical systems.
24. Experience of working with single assessment process.
25. Experience of working in a community/in-patient setting for a minimum of 2 years post registration
Qualifications
Essential
26. Registered Allied Healthcare Professional with HCPC - Physiotherapist Occupational Therapist
27. Degree/ Diploma in Physiotherapy Occupational Therapy
28. Experience of recent Continuous Professional Development.
29. Degree/ Diploma in Physiotherapy Occupational Therapy
30. Relevant post graduate clinical training Elderly Long Term Conditions Respiratory Disease Cardiac Disease Trauma, Orthopaedics & End of Life
