An exciting and unique opportunity has arisen to be a part of the Mental Health Intensive Support Service in East Cheshire.
This is a newly established Service that provides specialist assessment, treatment, interventions & support, to help people recover from complex mental health problems & to regain the skills & confidence to live successfully in the community. The team are incorporating an open dialogue approach when working with patients and families.
This vacancy is fixed term for 12 months. Secondments would also be considered.
Key Responsibilities:
1. Support the MDT in providing intensive support to a small cohort of patients, ensuring high-quality levels of care to assist these patients in living independent lives.
2. Work alongside the Primary Worker to ensure effective and adaptive patient care with a strong focus on recovery, identifying relapse indicators and promoting independence.
3. Ensure all patients engage in the agreed care plan and encourage inclusion in local community services to promote daily living skills.
4. Participate actively in training of Care Staff/third-party organizations that will support the patient in the community.
5. Demonstrate excellent skills in risk management, adaptive care planning, and communication across a range of scenarios.
Qualifications:
Candidates are welcome to apply from a range of health or social work backgrounds, or similar experience; however, experience in working with complex patients is encouraged.
