Main Duties of the Job

* Team of 4 nurses and 5 HCAs supporting and delivering treatment room and chronic disease services
* Semi-rural dispensing practice in an expanding town with low-cost housing
* Long established teaching & training practice, providing opportunities for career development
* Consistently high QOF achiever
* Multi-disciplinary PCN team including Mental Health Practitioner, Social Prescribing Link Worker, Care Home Care Co-ordinator, MSK First Contact Practitioner and Pharmacist team supporting with medication reviews and clinical correspondence
