We are seeking an Allied Health Professional or Registered Nurse (Adult) to lead and manage the Hospice at Home Team, in order to deliver high quality Community End of Life Care. Main duties of the job
The Hospice at Home Team are experts in ensuring high quality End of Life care is delivered to patients and their families. They work independently, and as part of the wider health care team. The Hospice at Home Operational Lead role provides clinical leadership, operational management, and professional support to the team. This includes staff recruitment and retention, skill mix and team development.
1. Work with the Access and Community Nurse Specialist Operational Lead and Head of Community Nursing to develop the Community Nursing Strategy and set team objectives.
2. Manage, lead and develop the Hospice at Home Team to work efficiently and effectively in the delivery of high quality and progressive end of life care in line with the Hospice strategy and policies.
3. Contribute to the culture of leadership across the hospice.
4. Develop collaborative working relationships with other hospice teams and the wider community services to align services.
5. Be part of the Hospice at Home on Call/Out of hours rota with potential to join the Senior Clinical on Call rota.

Job responsibilities
Leadership and management:
1. Manage a team, lead by example and contribute to the culture of leadership across the organisation.

5. Develop and communicate standards and expected levels of performance.

9. Lead the evaluation of strategic aims in own delivery area.
Clinical practice:
1. Oversight of the team held clinical caseload of patients living in the community.
2. Ensure a standardized approach to caseload management.
3. Evaluate how best to measure impact on patient care.
4. Take and analyse a clinical history in a relevant succinct and logical manner.
5. Independently assess the patients needs, identify their priorities and work with the patient and family towards achieving them.
6. Demonstrate an in-depth knowledge of the options for achieving control of symptoms in the last weeks of life and ability to advise the primary health care team.
7. Demonstrate an ability to explore the personal resources of families/carers in order to facilitate where possible that patients are cared for in the place of their choice.
8. Support and coach carers at home in managing a relative whose condition is deteriorating.
9. Recognise the impact of caring for dying patients on yourself, internal and external colleagues and offer / seek support.
10. Maintain the competence to safely administer medication in the community including syringe pumps and other competencies in agreed areas.
11. Recognise and manage palliative care emergencies.
12. Support the team to bring closure to the clinician-patient relationship and provide for a safe transition to another care provider as appropriate.
13. Instigate and support MDT case review meetings.
14. Investigate and respond to complaints and concerns.
Person Specification
Qualifications
* Current HCPC/NMC registration
* Communication skills training, or willingness to undertake
* Valid UK driving licence
* Leadership training
* Mentoring training
* Independent prescribing
* Physical assessment and clinical reasoning, or willingness to undertake
Experience
* Significant experience of leadership and management
* Knowledge of clinical governance and evidence-based practice
* Knowledge of issues related to the provision of palliative care in the community
* Knowledge of safeguarding individuals who are at risk
* Knowledge of relevant digital systems e.g. EMIS
* Symptom management for patients at the end of life
* Advance Care Planning
* Experience of teaching.
