Job summary The practice nurse must provide and maintain a high standard in general practice nursing and chronic disease management for patients, which includes the diagnosis, monitoring and development of individual management plans, and agreeing these as appropriate with the patient and other health professionals. They will provide assessment, treatment, screening, health education services and advice to patients, while adhering to practice protocols, policies, and guidelines. The post-holder must provide and maintain the highest possible standards of nursing care for patients as well as provide nursing assistance to the doctors and other members of the team. They will do this independently and with nurse and GP colleagues.
