We are currently seeking a highly skilled and motivated Band 6 Psychiatric Liaison Nurse to join the team in one of the major mental health Trust in Midlands.
Description:
To use advanced nursing skills and knowledge to carry out physical and psycho-social assessments for the rough sleeping population
To provide advanced clinical / therapeutic treatments based on best possible evidence that will improve health outcomes for the rough sleeping population.
To undertake non-medical prescribing and to ensure that other duties in relation to medicines management meet required professional standards
To work in partnership with other community teams/voluntary organisations in order to ensure the seamless delivery of care to the rough sleeping population and Neighbourhood working to support transition
To act as an advocate for people who are rough sleeping, supporting patients own choices in regards to their care whilst promoting independence and self-care
To constantly champion the principles of self-care and patient empowerment.
Act as a mentor/supervisor as required for non-medical prescribers within the service
Attendance at leadership and service steering groups
To take the lead in the delivery of specified educational programmes, on topics such as patient assessment, acute nursing care
To proactively identify service users who would benefit from the rough sleeping health service
To be in regularly communication with all members of MDT team
To proactively develop and maintain close working relationships with the Wider Neighbourhood working including all relevant stakeholders
Provide training in dual diagnosis and mental health treatment to other professionals
Experience:
1. Experience, working in primary or urgent care and also experience working as an Advanced Nurse Practitioner.
2. Advanced theoretical and practical knowledge of managing a wide range of clinical conditions gained as a result of additional training and work experience in primary care
3. Experience of prioritising complex workloads.
4. Experience of applying relevant legislation and national guidance within nursing practice
5. Experience of working collaboratively in a multi-disciplinary team
