We have an exciting opportunity for a highly motivated and enthusiastic clinician to join Older Adult Community Treatment.
As a Band 6 Community Practitioner, you will coordinate mental health care and treatment for patients within our locality area with cognitive and functionally frail conditions, focusing on care homes. You will provide thorough assessment and treatment plans within NICE guidelines to a caseload of patients across Sunderland. You will be able to work autonomously and as part of a team.
Our Older Adults Pathway is a multi-disciplinary team with other functions available which can support complex patient needs such as our Day Hospital, Step Up Service, Behaviour Support Service, Physical health monitoring, and Care home liaison team.
It is expected that you will possess excellent communication and interpersonal skills and have the ability to manage your time effectively. You will work in a highly supported environment and will be provided with training opportunities to develop your knowledge and skills. Our service is currently provided from Monday to Friday 9 am - 5 pm (excluding Bank Holidays) with some out of hours flexibility 8 am - 8 pm to meet individual needs.
Main duties of the job
* To be responsible for the assessment and management of casework of service users who have substantial and complex needs.
* In collaboration with the service user and carer where appropriate, develop formulation-focused care plans utilizing a psychosocial approach, ensuring timely reviews to monitor progress and adapt care plans as necessary.
* To provide compassionate care that is based on empathy, kindness, respect, and dignity.
* To provide clinical and psychologically based interventions.
* Provide specialist advice to service users, carers, staff, statutory agencies, third sector partner agencies, and the public.
* Provide clinical supervision to staff as delegated by Clinical Lead and Pathway Manager.

Person Specification
Education and Qualification
* Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
* Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
* Proven track record of post-registration clinical experience relevant to the post.
* Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
* Up-to-date knowledge/experience of relevant legislation.
Knowledge and Experience
* 12 to 24 months band 5 qualified nurse experience dependent upon service need.
* Awareness and understanding of professional accountability, NMC standards, and other local and national initiatives.
Skills and Competencies
* Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
* Risk Assessment and Risk Management/Safety Planning skills.
* Demonstrate an understanding of effective & appropriate treatment options including a range of evidence-based clinical approaches as identified in N.I.C.E. Guidance.
* Demonstrate effective written and verbal communication skills.
* Evidence of a high level of professional, organisational, and leadership skills (self & others).
* Ability to adapt within a changing environment.
* Ability to work under pressure.
