Job overview
This role is an exciting opportunity for a registered band 7 mental health professional (Nurse, Social Worker, psychology or OT) to work in a dynamic way across primary and secondary care to bring together mental health pathways and support the understanding of mental health within primary care. The role will provide holistic assessment of services users linking people into the community as well as providing seamless transition into secondary care if it is needed. The role will also form part of multidisciplinary meetings held by GP Practices within primary care.
The primary care mental health service is expanding and is a key component to our community mental health transformation. It is envisaged that the role will closely link to the community transformation of mental health and the core team that is being established. The role will link to a specific primary care network and work closely with the practices aligned to that network. The successful candidate will be based in GP sites within the PCN during working hours.
Main duties of the job
-To actively promote better understanding and knowledge of mental health and wellbeing within primary care to enable the MDT to provide integrated and holistic offers of support for children and young people .
-To take a lead on providing specialist assessment of CYP with mental health problems within the primary care setting -To provide evidence based, time limited interventions where appropriate and within scope of professional practice and experience
-When required, to facilitate swift and seamless transition into other mental health services
-To work collaborative with CAMHS services to support effective transitions of service users
-To work collaboratively with CYP and their families to:
a. support shared decision-making around their care and support working on strengths and assets;
b. promote recovery and self care
-To inform and influence the ongoing development of new and innovative models of support within primary care and secondary mental health service

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
High Cost Area Supplement - Outer London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,551 to a maximum of £5,735 pro rata for part time).
