Job summary The Sirona Adult Learning Disability Health Service is a Centre of excellence for specialist learning disability health needs. We work across Bristol North Somerset and South Gloucestershire. Multidisciplinary teams enable our service users to access health services through direct care and liaison with other services. We offer support and advice on how to work with meeting the health needs of people who have a learning disability and how to make reasonable adjustments. Main duties of the job To support adults' with a Learning Disability (LD) who present with complex physical health needs, this may also include an element of behaviours that challenge and/or mental health needs. To work within the multidisciplinary team (MDT) towards a person centered approach to formulate, implement and evaluate appropriate care plans to improve health outcomes. Be responsible for undertaking specialist nursing assessments to support MDT intervention and person centered goals. To work as part of an MDT to develop, deliver and implement resources and training to mainstream services to promote inclusion and improved outcomes for people with LD.
