To work as an autonomous practitioner, responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The advanced nurse practitioner will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.

1. Receive correspondence from external agencies and process accordingly.
2. Scanning documents to the relevant software programme and logging them.
3. Filtering documents suitably according to need and passing to the relevant clinicians to read and action.
4. Initiating contact with and responding to requests from patients, team members and external agencies.
5. Photocopy documentation as required.
6. Input data into the patients healthcare records as necessary.
7. Manage all queries as necessary in an efficient manner.
8. Maintain a clean, tidy, effective working area at all times.
9. Support all clinical staff with general tasks as requested.
10. Completion of administrative tasks in relation to patient results and letters.
Person Specification
Qualifications
* Registered Nurse;
* Post graduate diploma or degree (Advanced Practice Qualification);
* Qualified Nurse Prescriber;
* Qualified Triage Nurse;
* Teaching Qualification;
* ALS, PALS;
Experience
* Experience working in a primary care environment;
* Experience in chronic disease management;
* Experience in prescribing and undertaking medication reviews;
* Experience working as a practice nurse or community nurse.
Clinical Knowledge
* Wound Care / Removal of sutures & staples;
* ECGs;
* Venepuncture;
* New patient medicals;
* Chaperone procedure;
* Requesting pathology tests and processing the results, advising patients accordingly;
* Diabetes;
* Asthma;
* CHD;
* Immunisations (routine, childhood and travel);
* Understands the importance of evidence based practice;
* Broad knowledge of clinical governance;
* Ability to record accurate clinical notes;
* Ability to work within own scope of practice and understanding when to refer to GPs;
* Knowledge of health promotion strategies;
* Understands the requirement for PGDs and associated policy;
* Knowledge of public health issues in the local area;
* Aware of issues within the wider health arena.
