Job Summary To perform as a general practitioner working independently and with other health care professionals to assess, diagnose and treat conditions of patients within a primary care setting. To work in a community and care home setting, with the PCN-wide clinical teams, community, and secondary care colleagues, providing acute home visits, complex frailty for the housebound and care home population and proactive enhanced care for our ageing and frail population. To carry out care home ward rounds, supported by the PCN Care Home nurses and pharmacists. To perform clinical and administrative work based on the SystmOne Clinical system. Balance direct clinical care with involvement in broader PCN initiatives. To provide expert professional advice to patients, carers and colleagues. To be professionally accountable and responsible for all elements of clinical practice in accordance with the GMC code of conduct and ensuring the safety of patients and the quality of their care. Main Responsibilities: All general duties and responsibilities associated with a GP working within primary care: Undertaking a variety of duties including visiting patients at home, surgery face to face consultations, telephone consultations and queries, prescription administration and management, general administration (referrals, pathology, reports, correspondence) in a timely fashion, ensuring high quality care. Making professional, autonomous decisions in relation to presenting medical problems, whether self-referred or referred from other healthcare workers within the organisation. Assessing the healthcare needs of patients with undifferentiated and undiagnosed problems. Screening patients for disease risk factors and early signs of illness. Developing care plans for health in consultation with patients and in line with current practice disease management protocols. Providing health education. Contribute to optimise PCN Investment and Impact Fund (quality improvement markers) Recording clear consultation notes to agreed standards. Collecting data for audit purposes and complying with QOF requirements. Delivering enhanced services. Compiling and issuing computer-generated acute and repeat prescriptions. Prescribing according to local and national guidelines. Supporting Extended Hours services, enhancing patient access Clinical Responsibilities: In accordance with the PCN/practice rotas, as agreed, the post-holder will be available to undertake a variety of duties including visiting patients at home, telephone consultations and queries, checking and signing/authorising repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation. Assessing the health care needs of patients with undifferentiated and undiagnosed problems. Providing counselling and health education. Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate. Recording clear and contemporaneous consultation notes to agreed standards. Collecting data for audit purposes. Expectation to complete a mandatory induction. Compiling and issuing computer-generated acute and repeat prescriptions. Prescribing in accordance with the prescribing formulary (or generically) whenever this is clinically appropriate. Recognise the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
