Haematology Cancer Care Coordinator

Job overview
An exciting opportunity has arisen for an enthusiastic and highly motivated individual to join our Haematology team within the Medical Division of the Wirral University Teaching Hospital NHS Trust. The post holder will be the key person identified for providing cancer information and support, enabling people to navigate the health and social care system and make choices that are best for their cancer and their life. The aim of the role is to ensure that people living with cancer can access the right support at the right time.
The post-holder will take an active role in contributing towards the development and implementation of cancer survivorship initiatives, enabling people affected by cancer to navigate the health and social care system and become self-supportive, i.e., coordinating health and well-being clinics to enable independence and support self-management as appropriate.
The post holder will coordinate care by providing a single point of access for cancer patients, including rapid re-entry into the system for those identified as having urgent or specialist needs.

Qualifications, Specific Experience & Training
* NVQ level 3 qualification in Health or Social Care or equivalent experience
* Excellent computer/keyboard skills, ECDL or equivalent, proficient in MS Office
* Counselling qualification
* Basic Cancer Course
* Experience coordinating a patient workload
* Additional training to Foundation degree/diploma level or equivalent through short courses
* 5 GCSE qualifications with C or above including Maths and English
Knowledge and Skills
* Minimum 3 years’ experience of working in a health or social care setting
* Minimum 2 years’ experience of working with cancer patients
* Awareness of the individual needs of people affected by cancer
* Experience of responding to the support and information needs of others
