Job summary This is an exciting opportunity for an enthusiastic, motivated individual to join Fareham Community Mental Health Team. We are a supportive and busy multi-disciplinary team, which provides care co-ordination and recovery focused care for individuals with complex and challenging mental health needs. You will have an induction, regular supervision and on the job training alongside mandatory and statutory training. You will need to be able to work autonomously in the community and use your own initiative under the guidance of qualified staff. You will work closely with the teams care co-ordinators to deliver effective and competent recovery focused care to adults with severe mental health needs, both in their own homes and in the local community. This will include interventions such as graded exposure and anxiety management. We are looking to appoint a highly motivated, enthusiastic, flexible and resourceful individual who is committed to providing support and assistance to service users in their recovery. You will ideally have experience of working with adults with mental health needs and an interest in working with service users in a community setting. Car driver and use of car within work hours is essential. Main duties of the job To provide physical and psychological care to service users in accordance with the specified plan of clinical care and in line with the recovery model to service users who live in any part of the catchment area.
