Salary: £21,694 before probation £22,934 after successful completion of probation (£24,088 FTE) Hours: 35 hours Job type: Permanent About the role We are seeking a compassionate and dedicated Active Monitoring Practitioner to join our team within South Warwickshire. Join us in making a real impact on our community About the service Active Monitoring is a psycho-educational programme designed to provide early intervention provision within the Primary Care arena. With growing demands on Therapeutic Services resulting in increasing waiting lists it is getting more difficult for people to access the help they need in time. This role will involve working directly from a GP surgery delivering the Active Monitoring programme, the service complements/replaces the watchful waiting function and proactively works with individuals on agreed pathways to support early intervention via a facilitated self-help model. Please note South Warwickshire and Worcestershire Mind has recently merged with Coventry and Warwickshire Mind. You will be employed by Coventry and Warwickshire Mind but working for South Warwickshire and Worcestershire Mind. Key responsibilities Empower individuals: Work directly with clients to overcome early signs of mental health distress, such as anxiety, depression, and stress. Deliver tailored support: Utilise a facilitated self-help model to guide clients through personalised support plans. Collaborate with partners : Work closely with GPs and other healthcare professionals to ensure seamless care. Manage caseload: Efficiently manage a caseload of Active Monitoring clients. Contribute to service development: Provide valuable insights to enhance the Active Monitoring program. Essential requirements Experience: 12 months of experience working face-to-face with individuals experiencing mental health difficulties. Qualifications: Level 3 in Health & Social Care or equivalent. Knowledge: Strong understanding of mental health, social care, and risk assessment. Skills: Excellent communication, interpersonal, and organisational skills. Passion: A genuine commitment to improving mental health and well-being.
