The post holder will work within the Torfaen Memory Assessment Service. The post holder is a senior member of the Mental Health Team and will be responsible for the initial screening and nurse-led follow-up of service users who are experiencing difficulties with their memory and/or a diagnosis of Dementia.
They will work collaboratively across a wide range of multi-disciplinary services and agencies to provide effective delivery of health and social care, supporting a programme of recovery and social inclusion within the area.
The post holder will also be involved in the continuing development of this service within Torfaen and may also supervise junior staff.
Main duties of the job
The post holder will ensure the provision of a person-centred, efficient, cost-effective service for people referred to the Memory Assessment team with cognitive changes or concerns. This will include a supportive, person-centred and educational approach to both the service user and their family/carers. In addition, the role requires the post holder to respond and proactively assess individuals as deemed clinically appropriate.
The post holder will be working across the Older Adult Memory Assessment Service. They will be facilitating and coordinating the timely and effective assessment/discharge/transfer of patients by utilising clinical, managerial and communication skills. The post holder will be responsible for the timely assessment and management of patients who have complex health and social needs, related to and because of memory difficulty.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
