What you will do: Work as part of the multi-disciplinary memory assessment service team, independently assessing and planning towards diagnosis. Utilise your specialist knowledge of dementia strategies and interventions to provide effective treatment. Refer patients to other in-service agencies or external services as necessary, ensuring comprehensive care delivery. Confidently identify risk factors and take appropriate actions to mitigate them. Facilitate seamless communication between GPs, LCFT services, and non-statutory services to uphold high standards of patient care. Offer mentoring and clinical supervision for other staff and trainees. Embody and actively promote the Trust values within the service, adhering to NMC and HPC codes of conduct. Assist the team manager in delivering effective leadership, ensuring that patient and carer assessments, formulations, and care planning are consistently high in quality.
