Job summary To provide clinical excellence, leadership, innovation and facilitated learning, the post holder will have responsibility for leading and developing this speciality service to provide equitable care to patients presenting with urological cancers and act as a resource to carers, Multi-disciplinary colleagues and the primary health care team. Main duties of the job The post holder will manage a supervised caseload/client group providing highly specialist expert nursing advice, information and support to patients with urological cancer and their families at all stages of their disease and in particular whilst they are being cared for at South Tees NHS Hospitals Foundation Trust thus ensuring a smooth transition of care from primary surgery through to radiotherapy/chemotherapy and into the community, to discharge or death, thereby establishing links between the acute and primary care group, supporting both patient and carers. The post holder will liaise with services within South Tees Hospitals NHS Trust and County Durham and Darlington NHS Foundation Trust. They will liaise with other key partners such as Macmillan Cancer Support, Northern Cancer Alliance, Clinical Commissioning Groups, Primary Care Trusts, Social Services and Hospices to enable and ensure quality service provision for all patients under their care. They will provide assistance, support and health promotion advice to other staff, whilst improving clinical practice, enhancing service provision and streamlining patient focused care. The post holder will be an active core member of the multi-disciplinary team and network with other professionals, locally and nationally for the purpose of developing practice, research and audit, development of standardised patient information, and clinical supervision.
