Key Responsibilities: Case Management – Coordinate, plan, and oversee care packages for children with complex health needs. Experience in Complex Care – Previous experience working with children with complex health needs. Strong Leadership Skills – Ability to lead, manage, and develop a team of care staff. Case Management Expertise – Proven experience in managing care pathways and ensuring continuity of care.
