Job summary
NHS North West London ICB has identified an exciting opportunity for a number of full time permanent CHC Assessor positions within the Continuing Healthcare Team, based across the North West London area undertaking face to face assessments and reviews.
North West London ICB is looking to appoint a Clinical Assessor; the post holder will report to the Senior Clinical Assessor. The post holder is required to have a sound track record of delivering daily operational services ideally from an acute and/or community background. Additional experience of care homes and domiciliary care would be beneficial to the role.
Main responsibilities
The post holder will:
1. Be responsible for leading on the key operational day-to-day areas of work relating to CHC for adults.
2. Ensure that the CHC service is fully compliant with and follows the national frameworks.
3. Undertake CHC assessments and reviews of adults in receipt of CHC and FNC funding.
4. Support joint working with the LA ICBs and clinical teams within North West London.
5. As an experienced nurse or AHP, have a working knowledge of the Mental Capacity Act, Safeguarding, and experience of undertaking assessment and review.
6. Support service users to access Personal Health Budgets and undertake support plans.

Knowledge, Training and Experience
Essential
1. Relevant degree/professional qualification in Nursing (RGN, RNLD, RMN, Social Worker, AHP).
2. Evidence of continuous professional development.
3. Educated to degree level or equivalent level of experience of working at a senior level in a specialist area.
4. Experience gained over a number of years in a complex, multi-disciplinary environment.
5. Experience of case management with service users who have complex health needs.
6. A clear knowledge and understanding of The National Framework for NHS Continuing Health Care and its application in practice.
7. An understanding of the Mental Capacity Act 2005 and its application.
8. An understanding of commissioning for complex health needs in the context of NHS Continuing Healthcare.
9. A good level of working knowledge of the relevant legislation and theories underpinning the provision of services to the relevant client group.
10. A clear knowledge and understanding of service provision available locally, regionally, and nationally for the client group.
11. Understanding of relevant current NHS and social care market mechanisms.
12. Good working knowledge of adult safeguarding.
13. Awareness of Data Protection and confidentiality issues.
14. Effective interpersonal and negotiation skills enabling effective communication with clients, other professionals, colleagues, partner agencies, providers, and carers.
15. Excellent written and verbal communication including providing and receiving complex, sensitive, or contentious information.
16. Excellent time management skills.
17. Ability to prioritise and to work effectively on own initiative.
18. Ability to work in hostile environments and deal with conflict on a regular basis.
19. Understanding of the commissioning/procurement process for adult continuing health care packages.
