We are looking for a dynamic and patient-focused individual to join our Community Therapy Team.
You will work alongside the Locality Manager and Deputy Locality Manager to provide leadership, managing your team, ensuring patient flow, and identifying and taking forward quality improvement.
You will work as part of the Integrated Community Team (ICT) pathways to prevent inappropriate hospital admissions and to provide advanced holistic assessment/diagnostics, support, treatment, and rehabilitation of patients in their own home.
You will work closely with the Lead Nurse, Clinical Lead, and other members of the Senior Team to continue to develop the service. You will be required to travel to various locations within Hertfordshire Community NHS Trust to attend meetings that cannot be held virtually.
You will provide clinical leadership within the unit, ensuring all members of the therapy team deliver high-quality clinical care within the scope of professional practice and work within the competency framework.
You will support the therapists with managing their caseload, goal setting, and meeting the estimated discharge dates to maintain patient flow through the unit.
You will support and continually develop your therapy team, encouraging training, managing sickness, addressing performance issues, and undertaking recruitment of new staff.
Through audits and feedback, you will work with the locality manager, Deputy Locality Manager, clinical quality lead, and senior operational team to take forward continuous improvement.
A big thank you for considering joining us at Hertfordshire Community NHS Trust. You’ll find it a great place to work with many benefits offered to you as a member of our staff.
We put our patients, their families, and carers at the centre of our vision - Outstanding Services and Healthier Communities. Our staff are proud to be Innovative, Caring, and Agile in their work to help achieve the Trust vision.
We welcome diversity in our workforce and are interested in applicants from all backgrounds and ages.
You will support your team with the day-to-day management of their patients, working alongside the rest of the MDT to ensure patients achieve their goals.
In managing the therapy team you will monitor performance: addressing issues as they arise, complete appraisals, manage sickness, and support compliance with mandatory training.
You will work with the Locality Manager and Deputy Locality Manager to monitor performance of the unit and continue to implement identified changes to meet CQC requirements.
In addition to the basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a maximum payment of £1,915 per annum pro rata.
You must hold a full valid driving licence and access to a car for regular business use is essential (unless you have a disability as defined by the Equality Act 2010).
While it is not a legal requirement, Hertfordshire Community NHS Trust encourages and supports all staff to be vaccinated as this remains the best line of defence against COVID-19.
We are committed to supporting flexible working options for staff, wherever possible, and welcome discussions on flexible working patterns to support our staff in maintaining a healthy work-life balance.
