Main Duties of the Job
The post holder will work as a complex case manager, without direct supervision. You will be involved in delivering integration, working with GPs, Social services, and third sector organisations.
You will provide case management, facilitate early discharge from the hospital, prevent unnecessary/unplanned hospital admissions where appropriate, and support healthcare closer to home.
The service currently operates from 09:00 - 17:00hrs, Monday to Friday. The post holder will be expected to support their team, department, and organisation to achieve the Trust's Values in their day-to-day work. The post holder will be a Registered Nurse, with at least 2 years of community experience managing a complex caseload of patients.
You will be responsible for a caseload of patients with complex health and social care needs. You will work autonomously to case manage these clients and use advanced nursing skills, such as assessment and prescribing, to manage their conditions in the community.
They will work as part of the integrated care team, working within and across professional and organisational boundaries. Liaising with GPs, other professionals, and statutory and voluntary agencies, you will deliver individualised care to patients.

Starting with NELFT
NELFT places a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training, and the allocation of equipment. As part of the process, new starters will have the opportunity to meet the executive team, senior managers, and attend a number of drop-in sessions focusing on engagement, health and wellbeing, and key processes. The induction will be held at our head office in Rainham, Essex.
High Cost Area Supplement - London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,551 and a maximum payment of £5,735 per annum pro rata).

* Will utilize a range of quality improvement tools to review professional practice and initiate and evaluate changes to practice when required.
* Will access Clinical Supervision and professional support from sources agreed with the line manager to support critical reflection and safe practice.
* Will support and supervise colleagues and provide targeted education, training, and development as required.
* Will engage positively in performance management and participate in the review and development of their Personal Development Plan as required.
Will demonstrate effective clinical and professional leadership in regards to developing the strategy for the development of services surrounding Long Term Conditions.
