Job Summary
This is a fixed term/secondment vacancy until 31/03/2026.

An exciting opportunity has arisen to work as an Assistant Practitioner within the Generic Health Visiting team in Carmarthenshire. As an Assistant Practitioner, you will be working as part of a skill mix team with families in their homes or community settings, as delegated by the Health Visitor to improve the overall wellbeing and health of children and families.
To support the best possible outcomes for children and families, the Assistant Practitioner will deliver evidence-based strategies and key messages from the Healthy Child Wales Programme.
Main duties of the job
1. Undertake home visits to children/families and implement a planned programme of care, reporting progress/changes to the delegating Registered Practitioner.
2. Prepare information packs, make appointments, and participate in child health clinics under supervision.
3. Give appropriate advice to families regarding childcare, e.g., nutrition, sleep problems, and behaviour.
4. Follow up with parents/children who default appointments.
5. Reinforce key public health messages to children and parents/carers provided by the relevant practitioner.
6. Give advice on issues of safety in the home and the local community.
7. Facilitate a range of group work.
8. Introduce families to the range of local support that is available to them, i.e., Family Centres, Playgroups, Mother & Toddler Groups. Support the introduction of the family to these groups.
9. Undertake a range of clerical duties to support the team.
10. Report and record any concerns/complaints or issues to the Registered Practitioner.
11. Answer the telephone and ensure the information is received by the relevant staff.

Person Specification
Essential Qualifications and/or Knowledge
* Good standard of education, GCSE English and Maths (Grade A-C) or Level II equivalent qualification.
* NVQ/QCF Level 4 or equivalent level of knowledge or NVQ/QCF Level 3 PLUS Additional training to Diploma level or equivalent or Equivalent through short courses and experience and equivalent competencies achieved and evidenced through accredited courses.
Desirable
* Community experience.
Essential Experience
* Experience of working in Health Visiting/School Nursing/Children's social care or education.
Desirable Language Skills
* Welsh speaker (Level 1).
