Job summary Fixed Term for 24 months Until due to Funding An exciting opportunity to develop a Frailty Nurse Service for patients living in North Powys has become available. The post is a 2 year fixed-term contract working in rural and urban community settings covering Montgomery, Newtown, Llanfair Caereinion and Welshpool. The role will coordinate and provide essential additional support to some of our most frail patients, supporting the development of expert frailty services in the community. The role demands thorough clinical assessment skills and a person-centred approach to formulate personalised comprehensive care plans which aim to keep patients at home for longer, reduce acute admissions and prevent hospital re-admission. The postholder will be an autonomous clinician who will use their experience and expertise of working with frail elderly patients in a community setting, to coordinate the provision of care for our frail patients. A key member of the Frailty team, you will have proven clinical and communication skills, be confident in working alone, and experienced in providing a holistic and comprehensive clinical assessment of your patients. Your role will be vital in communicating the needs and wishes of the patient to those involved in their care. With this being a new role, you will work closely with a senior Band 8a nurse, supporting the development of the Frailty Nurse led services in North Powys. Main duties of the job Provide expert advice and specialist nursing knowledge to people living in North Powys with severe frailty, their families, and carers. Work with families in clinic and community settings. Provide comprehensive liaison consultation to other professional bodies and agencies. Use own initiative, make clinical decisions, and provide clinical direction for the Primary Care and Community Frailty team. Adhere to own professional body statutory requirements. Develop plans to ensure those with additional complex needs are enabled to access health promotion initiatives and health care effectively and appropriately. Work in close collaboration with Public Health Wales, Health Champions, and Primary Care to develop initiatives to identify and effectively respond to the health needs of people with severe frailty to improve health literacy, health outcomes and enhance the experience of healthcare services for service users and their families and carers. Responsible for the continual development of the Frailty Team ensuring that service delivery is of a high professional standard in line with Health Board policies and procedures. Responsible for reviewing all policies in relation to the Primary and Community Care Frailty Nurse service. Responsible for leading on the improvement Cymru action plan agenda. This post will comprise of a clinical caseload, educational duties and provide clinical direction for the Frailty team.
