Wirral Ark is looking for an experienced Case Management Worker with the following qualifications:
* Experience of working in a homeless hostel
* Experience of working in homeless services
* Experience of risk assessment and support planning
* Experience of case management, managing a case load and working with clients to progress
* Experience of working on shifts (8am-3.30 / 1-9pm)
Job Purpose
Working in Mary Cole House (MCH) to assist in the provision of effective, high quality support services to vulnerable, homeless men and women who may have needs such as offending behaviour, substance misuse problems, mental health issues, and who are vulnerable to rough sleeping, with the aim of promoting independent living.
Duties
Support Planning and Risk Assessing
* Carry out new client welcomes, inducting clients into the service
* Ensure that all voids are prepared and re-let in a timely fashion
* Maximise income receivable from rents and service charges
* Ensure that clients have a full understanding of their rights and responsibilities during their stay
* Support clients to achieve their goals and targets as defined/agreed within their support plans and support contracts
* Engage with reflective sessions
* Ensure the continuation of consistent support
* Manage, update and communicate client risk assessments in line with Wirral Ark policies and procedures
* Ensure clients shape their own support and take part in activities
* Hold planned case management sessions regularly with clients using a person-centred flexible approach
* Assess support needs via Outcomes Star assessments, and plan support around individual needs
* Regularly review and develop mutually agreed support plans with clients and any other professionals involved in the support of those individuals
* Ensure up-to-date records are kept on clients' files
* Conduct regular building and room checks and support clients in the management of their room and relationships with others
* Participate in the planning and delivery of activities
* Support claims on behalf of clients for housing and other welfare benefits
* Provide information and advice to clients in accordance with their support plans
* Motivate and challenge clients so they realise their goals
* Assist with securing permanent move-on to appropriate accommodation
* Liaise between clients and external agencies regarding the provision of specialised support when appropriate
* Encourage involvement and participation among clients in the running and shaping of the service
* Deal promptly with all complaints and protection from abuse disclosures
* Provide clear, accurate and good quality information in respect of all clients in line with agreed procedures
* Demonstrate creativity in overcoming obstacles and a commitment to working with and providing support to vulnerable people
Health and Safety
* Liaise with Emergency Services; Fire, Police and Medical
* Ensure at all times the safety of yourself and others on the premises
* Conduct routine Health & Safety inspections throughout the building
* Report incidents in line with Wirral Ark licence agreement
* Ensure that all communal and work areas are clean and tidy
Teamwork
* Attend to all enquiries and pass messages onto relevant people
* Develop excellent and robust relationships with referring agencies, statutory services, and wider stakeholders
* Contribute to the formulation of policy and procedures in relation to support services
* Attend and participate in regular team meetings to ensure good communication
General
* Have a flexible and positive attitude towards all team members

* Undertake own administrative services and report writing

Person Specification
For this role, the skills, qualifications, and experience listed below are important:
* Min. Level 2 NVQ/Diploma in Housing, Health & Social Care, Advice & Guidance or other relevant qualification (D)

* Excellent communication and interpersonal skills (E)
