An exciting opportunity has arisen for a highly motivated, enthusiastic and experienced nurse to join our structural heart team. The successful applicant must have relevant experience in cardiology, be able to demonstrate excellent communication skills, good time management, be able to work well under pressure both within a team and independently and work flexibly to the needs of the service.
Main duties of the job
1. Provide independent nurse-led telephone consultation on a daily basis, triaging, assessing and advising on symptom management appropriately for patients, significant others and other health care professionals, within and external to the Trust.
2. Carry out a nurse led preoperative clinic to ensure patients are worked up appropriately and fit to undergo valve intervention. Provide education to both patients and their relatives regarding their condition, admission, and procedure, and after care.
3. Support consultants in regular Structural clinics, assessing patients' frailty, organising tests and ensuring they are on the correct pathway.
4. Manage own patient workload in parallel to consultant in out-patient clinics. Following up and actioning results of investigations.
5. Orchestrate patient transfers from external trusts and review inpatients on the ward to be able to provide clinical nursing information to the team and MDT.
6. Communicate with relatives on behalf of the patient, gaining verbal consent to do so from the patient wherever possible.
7. Work as part of the multi-disciplinary team (MDT), liaising with consultants, registrars, GP's, Cardiac Physiologists, Nurse Practitioners, Heart Failure team, Cardiac Rehabilitation team, Coronary Care Unit staff and secretarial/support staff, all to achieve a co-ordinated, comprehensive service to patients requiring Structural Heart intervention.
