Job summary
The post holder is responsible for the delivery of practice nursing services to the practice population. Supported by the Nurse Manager, they will deliver care within the boundaries of their role, working within practice protocols, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. The duties will include all tasks normally undertaken by an experienced RN and in addition any roles agreed between the nurse and the doctors as appropriate, having regard to current training. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of practice policies and procedures and will also need to have an understanding of the public health profile and population needs in order to be proactive in ensuring services are met.
1. Carry out annual reviews and management for all long-term conditions, maintaining the annual recall system.
2. Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs
3. Following agreed clinical protocols with referral to senior nurses or GPs as appropriate assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being.
4. Deliver opportunistic health promotion using opportunities such as new-patient medicals
5. Cytology- Support and manage health needs of women presenting for cervical cytology consultations.
Main duties of the job
6. Diabetic knowledge is desirable.
7. Family Planning - Support and advise women requesting information relating to family planning needs and contraception.
8. Travel Clinic - Offer travel health advice by providing comprehensive advice to patients prior to travel including; vaccinations and medicines, safe sex, food hygiene, sun protection.
9. Able to document Consultations according to NMC guidelines
10. Ensures awareness of statutory and local clinical protection procedures, including systems of referral. Ability to recognise signs and symptoms of child abuse.
11. Routine immunisations/Childhood immunisations - Implement and participate in vaccination and immunisation programmes for both adults and children and ensure vaccines are administered under patient group directions.
12. Able to recognize and manage anaphylaxis according to current UK guidelines.
13. Able to perform Cardio-pulmonary resuscitation according to current UK guidelines.
14. Ability to obtain and document informed consent (either verbal or written).
15. Ensure infection control guidelines are maintained.
16. Chaperoning and assisting patients where appropriate who are being examined by another clinician
17. Assisting GPs with minor surgery, vasectomy and family planning clinics
18. Requesting pathology tests, for example urine culture, swabs
19. .Confidentially of information gained at work must be preserved at all times.
20. Assist in practice audits, CQC compliance evidence and QOF recalls.
21. Take lead on local incentive project relevant to role.
