What you will be doing
:
As a qualified Physical Health Nurse, you will lead and coordinate all aspects of the physical health needs of service users, while also supporting your colleagues and promoting good teamwork with the multidisciplinary team, such as GPs and other physical health departments.
You will be responsible for monitoring observations and assessments to identify any actions that need to be addressed and documented.
As a Physical Healthcare Nurse, you will be:
1. Adhering to the NMC Code of Conduct and facilitate the compliance with local, Trust and national standards of practice.
2. Developing and supporting physical health interventions used in the clinical area using knowledge of current best evidence-based practice.
3. Engaging actively in practice development, evaluation, audit activities relevant to physical healthcare and/or service areas to promote evidence-based practice.
4. Working collaboratively with primary health, developing and maintaining links with GP practices, and other physical health departments.
5. Monitoring and maintaining Physical Health Dashboards.

Where you will be working:
Castleholme Lodge is situated in Huddersfield with excellent transport links to the surrounding areas. It is undergoing a complete refurbishment to ensure best environment for service users and staff to thrive in.
Castleholme Lodge will be a 20 bed Care Home with nursing, providing a range of mental health services for adults with severe and enduring mental health difficulties. It will form part of a cluster of Elysium services across the Yorkshire and Midlands region that work closely together in support of each other.
