Duties Relevant checks for QOF and enhanced services as required. Obtaining medical information required by health monitoring targets and QOF domains. To input and complete medical data on the computer as required. Lifestyle health checks. Assisting in the assessment and surveillance of patients health and well-being. Sign posting to appropriate lifestyle change support agencies. Identify carers and encourage registration with carers support agency. Measurement of blood pressure, pulse, height and weight. Weight management with reference to protocol. Opportunistic BP and smoking assessments. Hypertension reviews. Urinalysis - Processing and management of laboratory samples requested by GPs and Nurses. Phlebotomy. Chaperoning duties. Flu, Pneumonia, Shingles and RSV vaccinations. B12 injections. Vaccine cold chain storage, monitoring and recording of fridge temperatures. Clinical waste and sharps disposal. Clearing and re-stocking consulting rooms. Maintaining levels of stock items and drugs and re-ordering when necessary. Maintain own clinic appointments, when necessary, by advance booking. Attending practice staff meetings, training and educational events as appropriate. To keep up to date with mandatory training. Undertake any other duties as the post requires.
