We are looking for a Band 5 Physiotherapist, preferably with some community experience to join our Community Therapy team.
You will have the opportunity to work within strong and supportive multi-disciplinary teams utilising your developing assessment and clinical reasoning skills. You will have the opportunity to take part in regular supervision, joint working and expert, evidence-based in-house training. Participation on weekend Rota is also required.
To be responsible for providing a professional Physiotherapy service and carrying an agreed caseload under the supervision of a Specialist Community Physiotherapist. To deliver a complex package of high-quality healthcare including assessment, treatment, and advice to patients with various needs in a variety of settings.
To provide Physiotherapy assessment and treatment to patients within a disciplinary format. To deliver a Physiotherapy service to patients within a variety of settings, requiring flexibility/adaptability in recognising the complexity of these environments.
You are expected to cover planned and urgent occupational therapy assessment and treatment as well as supporting Home first, discharge to assess pathway and rapid response.
Main Duties of the Job
* To be responsible for own caseload, evaluation of interventions and documentation alongside CSP standards.
* To act as a role model, ensuring high quality, professional standards are maintained at all times.
* To work within an integrated multi-professional team.
* To be accountable for assessing, interpreting, planning and implementing physiotherapy treatment and care to patients within professional guidelines, with support from specialist team members and the team coordinator.
* To undertake assessments, care planning and evaluation.
* Wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure that patients receive the appropriate care in the most appropriate setting.
* To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making.
* To support the Band 6 with the day-to-day planning of a high-quality Physiotherapy service.
* Organise and manage own time allocating work appropriately in order to achieve the patients' goals.
* To manage risks in diverse and sometimes challenging environments.
* To ensure that record keeping is to a consistently high standard.
* The post holder will interact with all members of nursing, therapy, medical and other allied professionals.

Essential Requirements
* MCSP Dip PT / BSc / BSc(Hons) PT
* Registered as a Physiotherapist with the Health and Care Professionals Council
* To possess a current, valid full driving licence and be suitably insured
* Evidence of continuing professional development
* Experience of applying research-based information to practice
* Competence in a wide range of physiotherapy skills
* I.T. literate
* Typing / data inputting skills for accessing patient information
* Ability to communicate with people from a wide range of backgrounds
* Ability to work flexibly and sensitively
* Wide range of interpersonal and communication skills
* Manual handling skills and experience
* Ability to sensitively inform service users on emotional issues.
* Organisational & time management skills
* Clinical reasoning skills
* The ability to identify and act upon adverse clinical signs

* Ability to move between sites in the Swindon and Wiltshire area
