About the role…
As an Enhanced Care Practitioner, you will be employed by Humankind to support higher need service users who are unable to progress their timeframe due to their multiple needs. Multiple needs include but are not limited to: chronic physical health, comorbidities, financial issues, memory/cognitive impairment, age, housing, limited social support, and other multi-agency support needs.
This is an interesting and rewarding role where you will have the flexibility to complete greater levels of outreach and go to service users to reduce barriers to engagement, support them to engage with specialist services, bring the intensive support and interventions to the individuals, and enhance positive outcomes through this flexible working.
The post holder will provide holistic support to adult service users to support their drug/alcohol treatment. Key roles and responsibilities include:
* Assess and engage with those individuals identified to have multiple needs who require a more flexible way of working, including outreach support to reduce identified barriers to engagement with mainstream support in a service setting due to their identified additional needs.
* Manage a service user caseload and review ongoing care and treatment, liaising closely with the full range of specialist workers within the multi-agency team, including family members and significant others, as appropriate.
* The role will include but is not restricted to conducting home visits, supporting clients to appointments with various services/specialisms, as well as meeting individuals in the community.
* Exercise excellent communication skills and therapeutic skills when dealing with Service Users, Carers, and Caring agencies.
* Work closely and in partnership with the multi-disciplinary team from across the Forward Leeds partnership and externally, providing advice and guidance to managers and other colleagues through attendance at reviews, team meetings, group supervision, and via telephone.
About you…
* The successful candidate must have a full clean UK driving licence and access to their own vehicle.
* You will have a passion for making a difference, and fair chances for all should be at the core of everything you do.
* You will have knowledge and understanding of interventions, including brief interventions, Motivational Interviewing, Psychosocial Interventions, Cognitive Behavioural Therapy, Harm reduction, and multi-disciplinary working.

Our vision is to break down the barriers that stop people getting the support they need to live a life they value. We tackle poverty and disadvantage in communities through mental health, drug and alcohol, housing, and related support.
We have almost 200 services around England and nearly 3,500 amazing staff and volunteers who run them. Every year our services support around 125,000 people.
Bringing together two big organisations is not a small task, and it’ll take a while to fully integrate our systems, processes, and approaches. That’s why you will notice the jobs here are still branded ‘Humankind’. Next year we’ll be fully integrating our recruitment systems, but for now, they are separate.
