Clinical Pharmacy in General Practice is part of an exciting programme of transformation to develop a new model of care which addresses our ambition to deliver person-centred, co-ordinated care across an integrated health economy.

The pharmacy role within primary care can pro-actively help patients stay safe and well and out of hospital, as well as helping reduce the demands on general practice.
Working together with Clinical Pharmacists, you will support patients to get the best outcomes from their medicines. You will work within a PCN promoting high quality and cost-effective prescribing practice.
The role provides a great opportunity to join a committed team as well as having access to national training through the NHSE Clinical Training Pathway and be part of a local network of clinical pharmacists/other technicians.
We are looking for candidates who are enthusiastic and adaptable to change, with good clinical and I.T. skills. Candidates should exhibit organisational skills and the ability to communicate effectively with a wide range of healthcare professionals and other stakeholders including patients, families, and carers.

* BTEC diploma level 3 in pharmaceutical science or NVQ level 3 Diploma in pharmacy service skills (or equivalent as recognised by GPHC)

* CPPE Primary care pharmacy education pathway in progress
* CPPE Primary care pharmacy education pathway completed
Knowledge
Essential criteria
* Significant post qualifying experience, working as a registered pharmacy technician in general practice, community or hospital pharmacy
* An appreciation of the nature of primary/secondary care prescribing, concepts of rational prescribing and strategies for the improvement of prescribing.
* Evidence of commitment to lifelong learning – CPD
Desirable criteria
* Understanding of general practice and related computer systems
* Evidence of patient centred approach to care
