We have an exciting opportunity for Band 7 Adult Nurse to join our team as a Nurse Practitioner within a busy Intermediate care unit. Intermediate care supports people to avoid going into hospital or residential care unnecessarily, facilitating people to be as independent as possible after a stay in hospital, or a period of not managing within their own home.
Our unit provides high quality care to patients requiring rehabilitation, to optimise health before returning to their own home. We work in collaboration with Physiotherapists, Occupational Therapists, and Social Workers to do this.

Main duties of the job
The post holder will work as a Nurse Practitioner within an established team of Nurse Practitioners and Therapists. They will be a skilled, independent practitioner, required to make autonomous, clinically led decisions. There will be a requirement to undertake clinical/physical examinations and formulate a plan of care based on these findings.
Completion of the history taking in physical assessment and demonstrable clinical skills are a significant part of this job role. If not already possessed, a willingness to undertake this is essential. This role also has a need for the right candidate to possess non-medical prescribing to work to the level of nurse practitioner. Again, the right candidate would be supported to achieve the v300 if not already achieved.
The team provides medical and therapy services to patients in Eden Court including:
* Comprehensive assessments
* Daily board Round
* Goal and care planning with patients and their carers
* Regular reviews by the team to ensure a needs led and responsive service. It is expected that stay in Eden Court will be approximately 6 weeks.

Job responsibilities
This role will require experience, as well as excellent clinical/managerial leadership. Advanced communication skills and the ability to act as an excellent role model are fundamental aspects of this role.
Acting as patient advocates when needed, an understanding of mental capacity and DOLS is an important part of this job.
The successful candidate will be expected to work alongside the existing Nurse Practitioner/Team Lead, as well as be part of the wider MDT ensuring high quality rehabilitation care is provided. A keen interest in rehabilitation is vital for this role.
The right candidate will be enthusiastic, dynamic, and able to work under pressure.
Awareness of computer systems and electronic patient records is advantageous.

* To ensure that the patient is the central focus of a streamlined patient journey ensuring optimal quality of care.
* Working as a nurse practitioner, provide a prompt response to calls from colleagues, prioritising importance and clinical need.
* Manage own caseload to include assessment, investigation, diagnosis, and treatment within defined parameters and training.
* Provide immediate care management of patient including communication of plan to patients and significant others.
* Review and refer to appropriate person, e.g. AHP, resident medical staff, specialist registrar, or critical care outreach team.
* Liaising with Patient Services Coordinator where necessary.
* Communicate within multidisciplinary team, ward/departmental staff, and/or out of hours team.
* Provide specialist education and training to departmental nursing staff, nursing and medical students.

* Registered Nurse (appropriate to branch)
* Current NMC registration
* Degree/Masters level study, or equivalent specialist study/experience e.g. Nurse Practitioner Qualification or equivalent.

* Has undertaken and completed Non-medical independent nurse prescribing course
Knowledge & Experience
Essential
* Meets Nursing and Midwifery Council (NMC) requirements for mentorship if relevant to role

* Has understanding/experience of adult/child safeguarding
