Hours: 37 hours per week
Location of post: Tonna Hospital with Hybrid Working Arrangements
This role receives a market pay supplement. The salary is (£41,223 - £45,403), full time equivalent including £3,000 supplement pro-rata) for a grade 9 Senior Social Worker or; (£35,834 - £39,223, full time equivalent including £1,000 supplement pro-rata) for a grade 8 Social Worker including newly qualified workers. This payment is reviewed on a 12 monthly basis.
The Community Mental Health Team based at Tonna Hospital are looking for a Social Worker with community mental health experience and a knowledge of current best practice in mental health.
The CMHT in the borough is a well-established multi-disciplinary teams, comprising of Social Workers, Community Psychiatric Nurses, Psychologist, Occupational Therapists, Consultant Psychiatric, Physical Healthcare Workers, Pharmacists and staff grade doctors. You will be joining a committed and supportive, motivated team, who work hard to deliver excellent standards in a positive working environment.
The CMHT work closely with primary care, Police, Health board, Drugs and Alcohol Services, voluntary and other statutory and voluntary organisations, supporting individuals with care and support needs across the county.
We have excellent partnership arrangements with the Crisis Resolution and Home Treatment team, Single Point of Access, inpatient care services, Local Primary Mental Health Services/ therapies Team, and Community Drug and Alcohol team, Housing Advice and other adult services teams, including the network disability team and community network teams.
Furthermore we work alongside our colleagues in Children and Young People Services, Child and Adolescent Mental health Services to support young people through the transition of services.
You would be expected to work within the legislative framework of the Mental Health Act 1983 (amended 2007) and the Mental Health Measure (Wales) 2010, along with demonstrating an understanding of how to work in a strength-based, outcome focused way.
A sound value base and excellent communication skills are also essential. Social Workers in the team are involved in undertaking Assessments, Reviews and Care Planning and need to have the enthusiasm to play an active role in delivering a high quality service to our service users.

Ideally you will have;
Applicants will need to be social work qualified and professionally registered as a social worker with Social Care Wales. Experience of working in a mental health and Approved Mental Health Professional status would also be advantageous.
You will need a knowledge and working practice of the Social Services and Wellbeing (Wales) Act 2014, to include outcome focused assessment and a working knowledge of the Mental Health Measures (2010), Continuing Health Care, the Mental Capacity Act 2005 including Deprivation of Liberty Safeguards.
