Making A Difference as one of the Advanced Practitioners on the Later Life, Adult Social Work Team.
An exciting opportunity has arisen for a suitable qualified Advanced Practitioner to join the management team of the Later Life Adult Social Work Team. The current management team consists of two Team Managers and three Advanced Practitioners The management work as a team to provide operational management to a busy social work team, the team report directly to the Head of Social Work, Later Life.
The Later Life team works with adults over the age of 67 years .The team is busy and the work complex and challenging, as part of the management team you will be expected to cover for the Team Manager when absent and to provide on-going support/cover to the other members of the management team who are responsible for managing the functions of their allocated team.
The role will be to assist and deputise for the Team Manager, to manage a team of Social Workers and Assessment and Review Workers to undertake social care assessments, safeguarding and the review of social care packages of care. This will assist to deliver outcome focused support plans for adults with eligible social care needs.
To support and undertake staff management, supporting wellbeing, training, and development, working with the standards of conduct, performance and ethics as described by Social Work England.
To drive good practice development and improvement in the service and ensure the safe and effective delivery of services to adults with eligible social care needs. To take a lead role supporting social work staff with complex cases.
This is a hybrid position with some home working; however, the post will require you to be based at our offices in St Helens and be able to attend the St Helens area regularly as part of your work
