An exciting opportunity has arisen for a Band 5 Physiotherapist with knowledge, skills and experience of intermediate care, rehabilitation and admission avoidance.

• 13 Intermediate Care beds (residential beds currently located at Elmhurst)
• Killelea House (36 Enhanced and residential IMC beds)
• Bury Rapid Community Response Service & Hospital at Home
• Home-based Intermediate Care and Reablement
The Intermediate Tier prides itself on providing strengths-based assessments and interventions to the people of Bury, helping them regain or attain as much independence as possible in their homes.
Our patients are at the heart of everything we do! We are looking for someone committed to being part of a healthcare team that delivers excellent care and support to our patients. To help us achieve this, you will be responsible for your own caseload, performing patient therapy assessments, providing a diagnosis, developing, implementing, and evaluating individualised holistic treatment programmes, and delivering a high standard of evidence-based care.
Within all our Intermediate Tier services, you will work within and have support from a multi-disciplinary team, follow appropriate pathways of care, and liaise with and refer to other agencies as appropriate.
We seek someone hardworking and motivated with excellent clinical, communication, organisational and time management skills. A flexible approach to day-to-day tasks is a must to meet the demands of the service. You will also need to travel across the service footprint, when necessary, in line with service needs, so access to transport is essential.
