Community Nurse Job Description
Main Duties
The Community Nurse will hold a caseload as a lead professional and care coordinator, working closely with senior practitioners in the team to provide direct patient contact for care planned interventions. This includes administering medications as appropriate or within role. The successful candidate will also be expected to work on the duty rota and take an active role in providing physical health care of patients on the caseload, in conjunction with the physical health lead and medics.
Interventions with patients may take place in the team building or in their own homes, therefore having access to personal transport is essential for this role.

Nursing applicants should hold NMC registration and have completed their preceptorship programme. Newly qualified Nurses or those due to qualify will be recruited via a separate process.
Due to the nature of the role, it is essential that applicants can travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.
Qualifications and Experience
1. Current professional registration with the Nursing & Midwifery Council RNMH or RNLD (RNSC - CAMHS only) - RGN (If appropriate)
2. Recognised practice assessor/mentor or work towards in an agreed timescale
3. Trained Clinical Supervisor (within 6 month probationary period)
4. Evidence of Continuing Professional Development
5. Evidence-based knowledge of best practice in caring for patients in the designated field
6. Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act)
7. Working with people with mental ill health/learning disabilities
