An informal chat about the role is welcomed Main duties of the job Following national and local guidelines, plan, organise, deliver and evaluate comprehensive health assessments to assess the complex needs of a Looked After Child or young person using evidence based knowledge to develop health plans toward improving the long term health outcomes for Looked After Children. The post holder needs to be an autonomous practitioner who develops and embraces a flexible range of working regarding the way services can be delivered to children and young people who are looked after toward achieving positive outcomes. Ensure that the health of Looked After Children is positively promoted, encouraged and supported. Empower children and young people to take responsibility for their own health and wellbeing. Work closely with all health professionals involved to ensure a comprehensive health assessment for children and young people is undertaken. Develop and undertake age appropriate health promotion advice and act as a resource providing health advice to children, carers and professionals. Establish robust lines of communication relating to health care between service users, professionals and agencies.
