Main duties of the job
As a Diabetes Specialist Nurse, you will:
1. Conduct advanced clinical assessments and create personalized management plans for your patients.
2. Interpret clinical signs, laboratory results, and other functional measures to initiate appropriate treatments and interventions.
3. Make autonomous, safe, and effective decisions to enhance patient health outcomes.
4. Communicate complex and sensitive information to patients, their families, and carers with empathy and clarity.
5. Organize and prioritize your workload, with flexibility to meet service needs and uphold quality standards.
6. Lead and mentor other healthcare professionals, enhancing their skills and contributing to community and primary care development.
7. Drive the adoption of evidence-based clinical practices and develop specialist pathways within the service.

Job responsibilities
The CSH Diabetes SpecialistNursing team provides specialist clinical advice and care, supporting thedelivery of all commissioned activities to adult patients within the North WestSurrey area and delivering an integrated model of diabetes services.
The role of the Diabetes Specialist Nurse service is to promote the health and wellbeing of people with diabetes enablingthem to achieve optimum glycaemic control through informedself-management. The post holder willprovide a link between GP and consultant led specialist teams and will provideeducation and support both to people with diabetes and those who care for them.
The post holder will beresponsible for facilitating improved diabetes care through the provision ofadvisory services, including telephone, video and face to face support withinpractices, clinics in an acute setting, nursing homes and patients' own homes.
The post holder will beresponsible for contributing to the ongoing development of clinical practice,research and standards of care within the service.

Essential
8. A minimum of 5 years experience as a Registered Nurse.
9. Evidence of working within a multidisciplinary team.
10. Experience in managing patients with diabetes, including both type 1 and type 2.
Desirable
11. Research or clinical audit experience.
12. Community healthcare experience.

13. The ability to clinically assess, deliver, and evaluate high-quality care.

23. First degree in nursing or equivalent level of experience. NMC Registered General Nurse. Evidence of certified continual professional development within diabetes.
