Project: Changing Futures – Bridge the Gap Trauma Informed Outreach
Purpose of Job: To work as an assertive outreach worker through a commissioned project under the Changing Futures agenda funded by the Department of Levelling Up, Housing and Communities and the National Lottery. The project is commissioned by Surrey County Council to deliver Bridge the Gap flexible, trauma informed outreach support to individuals coping with multiple disadvantage. The role will provide assertive engagement to complex clients through a trauma informed and holistic approach and include harm-reduction advice, motivation to change, benefits and accommodation support linked through a Team around the person approach led by SCC. This role will span across Surrey working with a small caseload. Based on a service user’s needs, there is an element of crisis response to this role so flexibility is needed between 8am and 8pm and weekend requirements.
Key Activities:
* To liaise and work with SCC Changing Futures team for Bridge the Gap and associated alliance partners in delivering the service and principles of the model.
* To represent Catalyst Support Bridge the Gap at Alliance meetings participating in feedback of evaluation outcomes.
* To attend professionals/TAP meetings in respect of Bridge the Gap clients and referrals as required.
* Provide advocacy to support clients develop their own narrative around their story.
* To facilitate joint working and a collaborative approach with other support services to tackle any barriers, including:
o Housing
o Health
o Social care
o Mental health
o Drug and alcohol
o Domestic Abuse
* Provide flexible, trauma informed assertive outreach in Surrey using a keywork approach for clients within Bridge the Gap.
* To conduct continuous risk assessment on clients’ needs and presenting behaviour and consider this risk in outreach approach.
* To conduct and record written assessments, support plans, wheel assessments in ECINS record system.
* To record client case notes factually and comprehensively after each client contact on ECINS.
* To support the National Changing Futures programme for both qualitative and quantitative outcome measures on DELTA.
* Transport clients in staff cars to appointments as required.
* Participate and engage in monthly team meetings to discuss complexity of clients on caseload and use the space for self-reflection and self-care.
* To adhere and work within the Catalyst Outreach Department risk assessment/safeguarding and all policies relating to the role.
* To participate in alliance training to support relationships and learning to develop continuous professional development.
* Engage with supervision and appraisals to support your mental health, wellbeing and delivery of the service provision.
* Participate in Bridge the Gap Clinical Psychological Supervision and Reflective Practice sessions.
* Adhere to incident management policy for the recording and monitoring of all incidents for clients, operations, health and safety and data.
* Represent Catalyst BTG in all aspects of your work.
Person Specification:
Qualifications and experience: Level 3 qualification in Health care or similar; lived experience relevant to the challenges faced by our beneficiaries.
Knowledge and skills:
* An understanding of confidentiality, consent, information sharing, in line with GDPR.
* A working knowledge of assertive outreach work, assessing and managing risk and Safeguarding, and policies and procedures supporting these.
* An ability to work with complexity (alcohol, homelessness, mental health issues) in the community.
* A good working knowledge of alcohol dependence and relevant education, harm reduction and treatment.
* Skilled in providing support within a multi-skilled/disciplinary approach within teams and with external organisations.
* A working knowledge of supporting clients to access services such as: hostel/housing, benefits, mental health, physical health, and treatment services.
* Strong communication and interpersonal skills.
* Good communication and written skills and a commitment to accurate and confidential record keeping on case management systems.
* Ability to interact effectively with the clients, colleagues and other professionals whilst retaining clear boundaries.
* Ability to work to all the policies, procedures and standards of the service and joint working arrangements with statutory and voluntary sector partners.
* A good understanding of personal limitations, ability to identify when to seek advice and support, and deal with issues in a professional manner.
Information Technology:
* Manual dexterity to use computer keyboard and mouse.
* Proficient in Microsoft Word, use of email, Outlook, and basic Excel skills.
* Ability to enter data onto a database as required by the Service.
General:
* A full driving licence and insurance for business use including transporting clients.
* Ability to work flexibly across operational hours and evenings where required.
* A passion for developing own skills/knowledge base through ongoing learning and development.
