As a Hospital to Home- Community Care Assistant you’ll play a vital role in delivering tailored rehabilitation support in the comfort and familiarity of customers’ homes. Key Responsibilities Deliver reliable, compassionate care tailored to each patients rehabilitation plan. Work collaboratively with care teams, hospital discharge teams, and occupational therapists to support patients recovery. Promote dignity, respect, and informed consent in all aspects of care delivery. Monitor and report customer progress toward rehabilitation goals promptly and professionally. Communicate effectively with multidisciplinary professionals to enhance rehabilitation outcomes. Your Role Assist occupational therapists in designing and implementing person-centred rehabilitation plans. Provide support with personal care, medication management, meal preparation, and household tasks. Safely use mobility aids and equipment while respecting patients dignity. Encourage social, physical, and mental activities to promote overall well-being and independence. Maintain detailed and confidential records of patients progress, changes in condition, and safeguarding concerns. What’s On Offer Salary: Competitive annual salary of £26,000. Mileage Allowance: 35p per mile for travel expenses. Training: Full training and development opportunities provided.
