To act as a ‘single point of referral’ into community mental health support from the Route One to Wellness partnership from health professionals in Primary, Secondary, and Local Authority care.
To assess and triage these referrals into services and activities provided by Orb Community Arts, the wider Route One to Wellness partnership, statutory and VCS organisations working within the developing HaRD Mental Health Hub & Spoke network, and other external agencies.

Salary Scale: NJC Pay scale 5 spine point 15 equivalent
Salary Range: £29,093 per annum FTE (based on 37.5 hrs full-time)
Location: Orb Community Enterprise, The Garage, Elephant and Castle Yard, Off High Street, Knaresborough. HG5 0EQ. The role will also require working at other Route One to Wellness, HaRD Mental Health Hub, and Better Connect partner premises and some travel to support onward participant referral or engagement with external agencies.

Working with: The Orb team, the Route One to Wellness Project Co-ordinator, and Route One to Wellness partner Keyworkers when appropriate.
Contract Start Date: to be agreed
Contract End Date: subject to ongoing funding
Holiday Entitlement: 28 days per year plus 8 bank holidays pro rata
Pension: automatic enrolment pension scheme provided in line with statutory regulation by National Employment Savings Trust (NEST): 5% employer’s contribution and 3% employee’s contributions rising in line with statutory requirements.
Probationary Period: the post is subject to a three-month probationary period.
Purpose of the Post
To provide 1:1 Keyworker support to a caseload of individuals at Orb Community Arts to engage with services, develop skills, confidence, and resilience, and progress into further learning, volunteering or employment opportunities.
Main Duties and Responsibilities
1. Referrals, Assessment, and Triaging (Route One to Wellness): Acting as a single point of contact for referrals into Route One to Wellness and HaRD Mental Health Hub & Spoke services from Primary Care (including GPs, Social Prescribers, and First Contact Mental Health Practitioners), Secondary Care (including the TEWV Harrogate and Ripon Integrate Care Teams and Mental Health Services for Older People), and the Local Authority (including NYC Health & Adult Services, and the Living Well team).
2. Liaising with referral partners to support knowledge of the Route One to Wellness and HaRD Mental Health Hub & Spoke offer and how to access it.
3. Providing a single Route One to Wellness/ HaRD Mental Health Hub wellbeing assessment that can be shared with Route One and Hub delivery partners.
4. Triaging individuals to appropriate services within Orb Community Arts, the wider Route One to Wellness partnership (including Claro Enterprises, Mind in Harrogate District, and Wellspring Therapy & Training) organisations working within the developing HaRD Mental Health Hub & Spoke network (including Community Anchor Organisations, TEWV and NYC), and other external agencies when appropriate.
5. Conducting short post engagement reviews with people supported through Route One to Wellness and the Mental Health Hubs, to provide understanding of impact and provide feedback to the original referring organisation.
6. Liaising with external agencies to ensure we have all the required information to be able to support an individual into services.
1. Engagement and Progression (Orb): Supporting a caseload of individuals to access services and interventions provided by Orb Community Arts.
2. Supporting these individuals to access other services and opportunities with Route One to Wellness, HaRD Mental Health Hub, and Better Connect partners, and other agencies when appropriate.
3. Providing these individuals with regular 1:1 meetings to support engagement, wellbeing, and progression.
4. Offering short-term low level wellbeing interventions that promote self-care, independence, and wider life skills.
5. Supporting individuals to set and achieve wellbeing and progression goals (including accessing volunteering, education, and employment opportunities).
6. Providing individuals with information, advice, guidance.
7. Supporting individuals to overcome barriers to progression such as housing issues, or accessing appropriate benefits.
8. Supporting individuals to undertake job preparation, work experience, work placements, volunteering, and education.
1. Networks: Supporting the development of peer-to-peer support opportunities for those engaging with Orb support.
2. Supporting people engaging with Orb, Route One to Wellness, and the HaRD Mental Health Hub support to influence services through participation in local service user forums and/or at system meetings organised by deliverers of mental health services.
3. Liaising with colleagues within the Route One partnership and those within the Mental Health Hub network from Primary, Secondary, Local Authority, and VCS partners in Multi-Disciplinary Team Meetings to ensure individuals receive linked up and holistic support.
4. Networking with and keeping abreast of local services and other sources of help relevant to individuals accessing support.
1. Administration: Collating and maintaining accurate records of individual engagement with Route One to Wellness and HaRD Mental Health Hub services.
2. Collating and maintaining accurate records of individual engagement with Orb services, including the requirements of funding partners such as the Better Connect partnership.
3. Sharing appropriate personal data with Route One to Wellness, HaRD Mental Health Hub, and Better Connect partners and referring organisation to ensure linked up holistic support.
4. Sharing anonymized data with the Route One to Wellness Project Coordinator and Better Connect to support project management and evidence impact.
1. Other Duties: Participating in supervisions, performance management, staff development and training.
2. Working towards individual and team targets.
3. Any other duties deemed reasonable in delivering the work of Orb Community Enterprise, and its partners at Route One to Wellness, the HaRD Mental Health Hubs, and Better Connect.
