Are you an experienced Mental Health Practitioner/Manager looking for an exciting new opportunity in South Somerset? For this post, we are looking to seek a Registered Mental Health Nurse or a Registered General Nurse with Mental Health experience. The role involves providing advanced clinical advice, leadership, and support to ensure the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis, and advanced decision-making skills. You will manage change, monitor and improve standards through supervision, evidence-based practice, clinical audit, research, and education. Additionally, you will promote and demonstrate best practice by integrating evidence into practice.
Maintain a clinical caseload within the Older Persons Community Mental Health Team of at least 50% of contractual hours, for which you will be personally accountable and professionally responsible. This Secondment post offers a rare opportunity to maintain and develop your clinical practice within a leadership and management capacity.
Main duties of the job
1. Advise and communicate as appropriate with acute hospitals, primary and social care, and community teams to ensure seamless continuity and transfer of care for patients between relevant health, social, and third sector agencies.
2. Establish, maintain, and effectively manage barriers to advanced communication with service users, carers, and professionals across health and social care, and education services.
3. Contribute to the development, implementation, and audit of protocols, guidelines, and policies for the service.
4. Participate in maintaining systems for risk assessment and minimisation, promoting a 'fair blame' culture for reporting and investigating near misses and incidents.
5. Contribute to investigations of incidents and complaints when required; participate in identifying lessons learnt and sharing knowledge across the organisation.
