Detailed job description and main responsibilities Patient Care Delivery Working autonomously - using an advanced level of specialized clinical expertise and judgment in assessment, planning and evaluating care. Communicate effectively with patients and their families in order to assess needs and to provide relevant support, information, education, advice and counselling. Ensure all patients have access to written and verbal information Attend relevant network meetings Collaborate with and refer to other relevant allied health professionals and medical colleagues as required. Offer expert opinion to colleagues and patients in secondary and primary care. Run nurse-led physical and virtual clinics taking full medical history, carrying out clinical examinations. Attend and proactively participate in relevant specialist nurse forums at Trust, Network and national level Effectively liaise with consultant and other members of the multi professional team to achieve a coordinated specialist service.
