Job Title: Peer Support Worker Grade & Salary: £25,750 FTE (£13,733 pro-rata) Hours of work: 24 hours per week Contract: Fixed term contract (2-years) Based at: Phoenix Rehab Unit, Springfield Hospital, 9 Lapidge Drive, London, SW17 0Y We are looking for a: We are seeking a Peer Support Worker who has personal lived experience of mental health challenges. This individual will act as a role model for service users, utilizing their own recovery journey to instil hope and resilience. By sharing their life experiences and the lessons learned throughout their recovery, they will motivate others. The Peer Support Worker will collaborate with the multidisciplinary team within the Unit, assisting in-patients in developing essential skills and coping mechanisms, while promoting overall wellness through strength based and non-directive conversations. They will provide both practical and emotional support, facilitate access to community resources to help individuals maintain their independence while ensuring their wellbeing. Additionally, this role includes supporting service users after discharge in the community for up to 12 weeks, aiding them in fostering independence within their local community, connecting with various agencies, enhancing life skills, providing guidance related to employment or educational opportunities and improving their ability to manage challenges effectively to prevent re-admission. This position requires building connections with service users through shared experiences of mental health challenges, making lived experience a crucial requirement for applicants. You will: Support patients whilst they are on the unit, working towards setting objectives to combat social isolation, identifying interests they may have and helping facilitate their engagement with community activities. Further to their discharge from hospital, you will support them for up to 12 weeks in the community which will include facilitating access to community activities around their interests, building skills to manage their independence, maintain their wellbeing as they settle back into the community and living independently after an Inpatient stay, also providing practical, emotional support as needed. Offer supports to represent their views and interests to other professionals and Identify with them how to access various supports in times of distress, various support networks within their area and signpost to advocacy services where appropriate. You will need to be a warm and welcoming "people person", be patient, resilient and open about your own experiences, have good communication skills and being able to feedback to the MDT in meetings. You will have an opportunity, to make a difference in people's lives, to contribute to the evaluation of the effectiveness of this service and to be part of the local Mind team supporting people with their mental health in South London. Promote understanding of the principles and practice of Peer Support and service user run services amongst staff and service users within the service. You will contribute in MDT meetings, Ward rounds and liaise with community teams and accommodation staff. Keep accurate records, using the relevant software, database, and contribute to the evaluation of the effectiveness of the service as required, to include ensuring that outcome measures along with patient notes are completed and recorded in a timely accurate way. You will be line managed by the Senior Peer Support Worker who you will work alongside and they will provide guidance, support, and regular supervisions. In their absence, you will be supported by the In-Reach Service Manager.
