We are looking for a highly motivated, adaptable individual to join our team on the Discharge Lounge, Integrated Hub at Huddersfield Royal Infirmary
The successful candidate will need to be able to use their own initiative and work well within a team.
The discharge lounge is a busy, pro active ever changing environment, diverse and challenging .

Main duties of the job
Transferring and accepting patients from every ward area, to the discharge lounge into the integrated hub.
Coordinating discharge planning of the client, liaising with care providers, care facilities and family members. Recognising the need for further community referrals to safeguard patient safety and create an effective discharge .
Completing patient education with regards to care providers, the discharge plan, medication management and any follow up arrangements.
Effective communication and excellent documentation on the electronic patient record
Whilst in our care, completing hygiene, nutrition, hydration, elimination and mobility needs. Engaging with the patients throughout their time in the hub., offering comfort, compassion and reassurance.

Qualifications & Training
Essential
1. NMC Registered Nurse in relevant field of practice
2. Practice Assessor/Practice Supervisor qualification or willingness to complete

3. Additional post-registration qualification relevant to field of practice
4. Evidence of continuing professional development
Knowledge, Experience & Experitise
Essential
5. Knowledge and skill to plan, deliver, and evaluate high quality, evidence-based nursing care to a group of patients within the speciality of the post
6. Experience of working with patients, and those that matter to them, from a range of diverse cultures and backgrounds
7. Experience of working as part of a team, with the ability to prioritise own workload.
8. IT literacy and ability to use NHS systems
