Clinical Case Manager - Experienced in Paediatrics & Adults

Full time: 37.5 hours per week

Shifts: choice of 8-4 or 9-5

Salary: up to £50,000 per annum (DOE)
We’re looking for a Clinical Case Manager with extensive experience in Inpatient Bedside Nursing for Adults & Paediatrics or extensive experience working with a healthcare insurance company to join the Case Management Team at HCA Healthcare UK.
As Clinical Case Manager, you will work closely with various healthcare professionals assessing, planning, implementing, and evaluating care options and services that meet the patients’ health needs and facilitating the coordination of proactive comprehensive discharge planning activities.
You will act as a resource/liaison between patient, hospital, physician, and third-party payers to optimise patients’ community treatment/services as required. You will work closely with third-party payers to keep them appraised of their patients’ condition and needs, obtaining authorization as required.
What you’ll bring:
* NMC Registered Nurse with substantial post-graduate experience (Adult registered with Paediatric Experience)
* Experience of utilisation review and extensive discharge planning experience essential
* Extensive bedside patient experience and/or extensive experience working with a healthcare insurance company
* Experience as a Senior Nurse/ Band 6 or above essential
* Management experience essential
* Advanced clinical assessment qualification desirable
* Paediatric experience
* Extensive knowledge of discharge planning and community resources essential
* Stakeholder management
* Ability to take initiative to work independently and within teams
* Excellent communication, organisation, and computer skills with a dynamic approach to problem-solving.
